*Physical Therapist – Home Health – Flexible Hybrid Pay (Points + Hourly)*
*Location: Pleasanton, CA*
*Compensation: $148,000–$160,000 annually*
*Schedule: Full-Time | Flexible Caseload | Field-Based*
*Overview*
*P3SG* is searching for an experienced and motivated *Physical Therapist* to serve patients throughout *Pleasanton and surrounding Tri‑Valley communities*. This opportunity offers a *hybrid pay model* (hourly + points) designed for predictable earnings while rewarding productivity and quality of care.
*What You’ll Do*
* Complete patient assessments and build individualized home‑based rehabilitation plans.
* Deliver evidence‑based PT interventions including mobility, strengthening, gait, balance, and functional safety training.
* Provide caregiver training and education to support patient progress.
* Communicate with physicians, case managers, and office staff regarding care updates.
* Document all therapy visits using our streamlined EMR system.
* Supervise PTAs in accordance with state requirements.
*Qualifications*
* Active California PT license required
* Graduate of an accredited PT program
* 1–2+ years PT experience (home health preferred)
* Current CPR certification
* Valid driver’s license and reliable, insured vehicle
* Strong communication & documentation
* Able to work independently with a flexible schedule
