*Overview*
Forest Park Home Health is seeking a dedicated Home Health Speech-Language Pathologist to support older adults in the comfort of their homes. This role focus on evaluating and treating communication, cognitive-communications, and swallowing disorders commonly seen in the geriatric population. The ideal candidate has experience serving older adults, understands age-related change affecting speech and swallowing, and demonstrates patience, empathy, and professionalism. This position offers an opportunity to positively impact patient independence, safety, and quality of life while working within a collaborative interdisciplinary care team.
*Duties*
* Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments for adult and geriatric patients in the home settings.
* Develop individualized, functional treatment plans tailored to patient goals, diagnoses, safety needs, and caregiver support, using evidence-based practices
* Provide direct therapy targeting communication, cognition, voice, fluency, and dysphagia to improve independence, safety, and quality of life.
* Collaborate closely with families, caregivers, physicians, nurses, and other healthcare professionals to ensure continuity of care and optimal patient outcome.
* Monitor patient progress regularly and adjust treatment strategies accordingly
* Educate patients and caregivers on safe swallowing strategies, home exercise programs, communication supports, and compensatory techniques.
* Maintain accurate documentation of assessments, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations
*Skills*
* Proven experience working with older adults in home health, skilled nursing, hospitals, or rehab settings.
* Strong knowledge of communication and swallowing disorders commonly associated with aging, neurological conditions, and chronic illness.
* Excellent patient care skills with the ability to build rapport with elders and their caregivers.
* Solid understanding of anatomy, physiology and medical conditions affecting speech, cognition and swallowing (e.g., stroke, dementia, Parkinson's Disease).
* Familiarity with Medicare guidelines, home health regulations, and healthcare documentations standards.
* Effective communication skills for collaborating with multidisciplinary teams and educating patients and family caregivers on therapy goals and strategies
Pay: $50.00 - $60.00 per hour
