*Essential Functions: *As a Registered Nurse (RN) in Home Health, you will:
* Provide and direct nursing care to patients at home in compliance with applicable laws, regulations, and agency policies.
* Coordinate the total plan of care with other healthcare professionals.
* Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources within a Point of Care setting.
*Responsibilities:*
* Provide clinical services within the scope of practice defined by state laws.
* Conduct initial and comprehensive nursing evaluation visits, determine the primary focus of care, and develop the plan of care within state-specific guidelines.
* Regularly evaluate and re-evaluate the patient's nursing needs.
* Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
* Initiate, develop, implement, and revise the plan of care in collaboration with physicians and other healthcare professionals involved in care.
*License Requirements:*
* Current driver's license and vehicle insurance, with access to a dependable vehicle or public transportation.
* Current CPR certification.
* Current RN License in the state of practice.
