We are actively hiring for the Northside Jacksonville area and looking for a motivated Physical Therapist to join our growing team.
*About the Role*
We are seeking a dedicated, compassionate, and skilled Physical Therapist who thrives in a patient-focused environment. The ideal candidate has experience across various settings—home health, outpatient, and inpatient care—and is passionate about helping patients improve mobility, reduce pain, and regain independence.
In this role, you will work directly with patients to create personalized treatment plans and provide hands-on therapeutic interventions throughout their rehabilitation journey.
*What You’ll Do*
* Perform comprehensive evaluations to assess patient mobility, strength, and functional limitations
* Develop customized treatment plans based on patient goals and clinical findings
* Provide therapeutic interventions such as manual therapy, exercise programs, and trigger point therapy
* Continuously monitor progress and adjust treatment plans as needed for the best outcomes
* Educate patients and families on home exercises, treatment expectations, and discharge planning
* Collaborate with an interdisciplinary healthcare team to ensure seamless, coordinated care
* Maintain accurate and timely documentation of evaluations, treatments, and outcomes
* Stay current with new physical therapy techniques, best practices, and clinical advancements
*What We’re Looking For*
* Degree in Physical Therapy from an accredited program
* Active state PT license
* Experience in home health, home community care, orthopedics, or acute care (preferred)
* Strong knowledge of physiology and evidence-based therapy principles
* Proficiency in outpatient rehabilitation and discharge planning
* Excellent communication and interpersonal skills
* Ability to work both independently and collaboratively within a healthcare team
