*PRIMARY FUNCTION* The Occupational Therapist plans and provides skilled occupational therapy services to the clients in their home in accordance with the Plan of Care and the Company’s policies and procedures. Performs functions which require substantial specialized knowledge, judgement and skill based upon the principles of psychological, biological and social services and must be able to make judgements accordingly. The Occupational Therapist reports directly to the Clinical Supervisor. *MAJOR RESPONSIBILITIES* 1. Performs initial and periodic assessments of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes. 2. Establishes reasonable treatment goals, keeps treatment care plans current, reassesses client and makes recommendations. 3.
