This role focuses on outreach and support to communities affected by Hepatitis C Virus (HCV), enhancing access to essential medical and social services. Utilizing a mobile healthcare model, this position plays a crucial role in our mission to reduce HCV spread and improving patient outcomes through personalized care and education. Responsibilities Conduct field-based health education on HCV prevention and treatment. Provide support and navigation services to patients accessing HCV care. Collaborate with healthcare providers to coordinate patient care plans. Collect and report data on patient outcomes and program effectiveness.
