Job Description Pay range is $72,800 - $86,660 / annually Position Summary Promotes healthy lifestyle in managing residents care experience through proper communication flow between the community’s onsite Medical Clinic, therapies, ancillary services, and community interdisciplinary team. Develops partnerships with service providers and agencies for resident referrals. Educates residents on available services and monitoring provisions of services. Responsible for fostering a continuum of care for residents. Serves as a liaison between families, residents and healthcare professionals as needed.
