JOB RESPONSIBILLITIES: Assists with admission process by taking initial vital signs, measurements, weights, orienting patient/family to unit, hospital facilities and services, and documenting per unit standards. Assists RN in delivery of patient care according to unit standards, policies and procedures, and patient’s plan of care. Contributes relevant and appropriate information to RN. Maintains confidentiality of patient information. Supports the Patient Bill of Rights. a. Participates in meeting and reinforcing patient’s educational needs and documents accordingly. b. Demonstrates knowledge of sterile technique. Under direction of RN, monitors and accurately records patient intake and output. Reports changes in patient’s status (e.g., changes in vital signs, level of consciousness, weight, mobility status, comfort level, feeding pattern or any unusual occurrences) to RN in a timely manner.
