Location/Department:  Hospital - Med Surg

24/pp - every other weekend evenings Friday, Sat, Sun 2p-10p

Job Summary:

To facilitate communication within the Med Surg Unit & provide information.

 Supervisory Responsibilities:  None

 Duties/Responsibilities:

  • Uses only approved abbreviations.
  • Understands patient safety goals & actions taken.
  • Demonstrates awareness of patient rights.
  • Uses 2 patient identifiers before giving treatments, demonstrates process.
  • Understands the spiritual aspects of patient care & can give examples of such.
  • Uses respect & dignity when dealing with death & dying.
  • Accurately reports to RN as warranted by condition.
  • Gives report to other providers as needed.
  • Reports sentinel events or adverse drug reactions or incidents as warranted.
  • Answers call lights promptly & courteously & routes information as needed to staff.
  • Acknowledges guests within 10 seconds of arrival to desk & greets them with a smile.
  • Provides directions to guest as requested.
  • Answers telephone within 3 rings & provides necessary information or relays messages to staff promptly.
  • Rounds on patients at a minimum of every 2 hours & more frequently as indicated.
  • Takes vitals including BP, T, RR, & HR as ordered & documents timely in EHR.
  • Weighs patients using appropriate technique & documents timely in EHR.
  • Records intake & output throughout the shift & documents timely in EHR.
  • Assists with or bathes patient with privacy & documents timely in EHR.
  • Provides mouth care with AM or HS care or as needed & documents timely in EHR.
  • Provides cath care at HS or as needed & documents timely in EHR.
  • Passes, sets up, & collects meal trays timely & documents in EHR.
  • Assists patients with meals as needed.
  • Passes snacks & refreshes ice/water at least once per 8-hour shift.
  • Assists with walking, turning, transferring using appropriate technique & documents timely in EHR.
  • Utilizes the correct bed type & mattress per patient condition.
  • Performs active or passive ROM using appropriate technique & documents timely in EHR.
  • Applies medical equipment using appropriate technique & documents timely in EHR.
  • Sets up patient rooms for admission.
  • Takes patients to car when discharged in wheelchair or as otherwise indicated.
  • Packages up patient belongings for discharge or transfer.
  • Transports patients to/from MS or from ESD as warranted.
  • Processes instruments/equipment per protocol & can state process.
  • Cleans patient care items as needed & keeps room clean & tidy.
  • Keeps grossly soiled linens separate from other linens to prevent contamination.
  • Maintains office equipment in good working order & notifies manager when repairs are required, takes out of service.
  • Notifies appropriate staff when patients are in isolation, keeps patient data screen up to date, kardex.

  • Aware of door alarms & security methods of MS/SCA.
  • Uses equipment safely & attends in-services as needed/offered.
  • Restocks supplies when needed to PAR levels.
  • Admits, transfers, & discharges patients from EHR & other forms promptly & accurately.
  • Prepares charts appropriately for new admissions & disassembles for discharges/transfers.
  • Places appropriate paperwork/strips on correct chart.
  • Orders weekly office supplies.

  • Effectively communicates both verbal & written.
  • Able to multi-task with accuracy & promptness.
  • Able to work with health care providers.
  • Able to operate basic office equipment & nurse call lights.
