Job Description

 

Daytop Village of NJ, Inc. is seeking a full-time Case Manager for our residential program located in Mendham, NJ, to join our high performance team.

 

This team player will be familiar with CYBER, TIER, Evidence Based Practices and Practice Based Evidence. He/she will obtain initial authorizations via CYBER and Perform Care within approved and designated timeframes. The Case Manager will facilitate treatment team meetings; coordinate client care with all internal departments and external stakeholders; will provide family contact, phone calls with stakeholders, review of recovery plan and phases, resolution of crisis, and discharge planning. The Case Manager will provide case management; facilitate group and educational counseling; direct care supervision; documentation in TIER within 24 hours of service; incorporate recommendations and develop Person Centered Recovery Plan; perform additional duties at the discretion of the Clinical Director or designee; serve as back-up for self-administration of medications when nurse is out; and serve as back-up for other Case Managers to facilitate reauthorizations and discharges at the direction of Clinical Director or designee. Qualifications

Desired candidate will have a minimum BA degree; Preferred CADC/MA degree.
